Five Wishes and More

Discussion of the importance of advance care planning and making one’s health care wishes known through use of Five Wishes and other planning tools.
 
Mary Crowe, LICSW, ACHP-SW, CDS is the Education Specialist for Care Dimensions (formerly Hospice of the North Shore & Greater Boston).  Over the last 12 years I have led the organization’s professional and community educational outreach, coordinating and delivering hundreds of programs each year for healthcare professionals, families and community groups.
I hold a Master’s degree in Social Work from Boston College Graduate School of Social Work, and post graduate certificates in gerontology and alcohol counseling. I also have a Facilitator Certification- Virtual Dementia Tour, Dementia Live Certified Facilitator, CARES Dementia Specialist (CDS), Certified in Alzheimer’s Association essentiALZ Plus Dementia Advanced Care, Dementia- Related Behavior, Activities of Daily Living and End of Life Dementia Care.  I am also an ELNEC End of Life Nursing Education Consortium- Core Trainer
I worked as a medical social worker for over 25 years in acute care, rehabilitation, skilled nursing and adult day health settings caring for individuals and families with advanced illness or at end of life. I am an Adjunct Professor at North Shore Community College and a Visiting Professor at Salem State University.

Adult Foster Care Awareness Day

The Massachusetts Council for Adult Foster Care (MCAFC) will visit the State House to educate legislators and their staff about the importance of Adult Foster Care to individuals and families throughout the Commonwealth.

The MCAFC is looking for as many people as possible to come and visit with legislators and their staff. We want them to meet the individuals, families, and friends of Adult Foster Care. We will share information and stories to spread awareness and to advocate for sufficient funding for the Adult Foster Care program.

Training: Care Transitions- Laurie Herndon

Laurie Herndon is a Geriatric Nurse Practitioner with more than 20 years clinical practice caring for older adults and families. She currently serves as the Director of Clinical Program Innovation at Seniorlink. In the last 10 years Laurie has developed expertise in area of care transitions first as project consultant on program designed to help nursing homes improve care transitions and then as a member of the Massachusetts Care Transitions Steering Committee. These experiences offered opportunity to train and collaborate with 100s of providers across the country and MA on this important topic.
This session will cover:

Strategies For How to Work with Caregivers to Improve Care Transitions

An MCAFC Breakfast and Lunch will be provided!
